On our website you can schedule a consultation yourself. For this we ask for your first name and surname, date of birth, gender, phone number, email address, the type of appointment and, if you wish, a short explanation. We use this data to schedule and confirm the appointment and to send you the medical questionnaire before the consultation.
To confirm your request, you receive a verification code by text message; your phone number is used for this. The appointment details are processed in Clinicminds, our clinic's scheduling and patient system, and after your consultation they become part of your medical record, to which the statutory retention periods apply.
For our website statistics, we measure that an appointment has been booked. No name, date of birth, contact details or type of treatment are passed on to measurement services.
